At Pathways to Promise Foundation, we believe that no family should navigate care systems alone.

Our Case Management & Care Coordination services are designed to walk alongside families every step of the way—from the first point of contact through reunification or permanency. Our trained case managers provide individualized support to help families:

Understand and complete court-ordered requirements Access community-based services, such as mental health care, housing support, substance use treatment, and parenting programs

Develop personalized case plans that reflect each family’s strengths, needs, and cultural values

Coordinate appointments, services, and progress tracking to ensure nothing falls through the cracks

Advocate for families within the child welfare system to help their voices be heard

We work in partnership with birth families, caregivers, and service providers to reduce barriers, build trust, and promote lasting change. Whether a family needs help locating resources, communicating with agencies, or managing the stress of the foster care process, Pathways to Promise Foundation is here to guide and support them every step of the way.
